Welcome to Barrera Dental! We are honored you have chosen us and want to make your visit as comfortable and easy as possible.
Your initial visit will include X-rays, thorough examination, consultation and treatment planning.
Please alert the office prior to your treatment if you have a medical condition such as artificial heart valves, joint replacements, or rheumatic fever.
Inform the office if you are taking any medications including blood thinners or medicine for osteoporosis (bisphosphonates)
In order for us to better serve you, please bring the following items with you to your visit.
Dental Insurance Card (if applicable)
List of Current Medications (including dosage information)
Contact information for physician treating your medical conditions
Please note that all minors (under the age of 18) must be accompanied by a parent/guardian during treatment planning.
If you are unable to attend your scheduled appointment, please notify our office at least 24 hours in advance.
